Süreğen Depresif Bozukluk Üzerine Kuramsal Bir Araştırma

Özet

Bu kuramsal derleme, Süreğen Depresif Bozukluğun (SDB) klinik özelliklerini, tarihsel gelişimini, tanı ölçütlerini, epidemiyolojik ve etiyolojik belirleyicilerini, komorbidite örüntülerini, risk ve koruyucu etmenlerini ve tedavi yaklaşımlarını sistematik biçimde incelemektedir. Kronik depresif belirtilerle seyreden ve bireylerin yaşam kalitesini uzun süreçte belirgin biçimde azaltan SDB, daha önce “distimi” olarak adlandırılan bozukluk ile kronik depresyonu kapsayacak şekilde, DSM-5’in 2013 yılında yayımlanmasıyla tek bir tanı kategorisi altında birleştirilmiştir. DSM-5-TR’ye göre Süreğen Depresif Bozukluk, yetişkin bireylerde en az iki yıl süreyle günün büyük bölümünde devam eden depresif duygudurumla karakterize edilmekte; bozukluğun ayırt edici özelliğinin, belirtilerin şiddetinden çok kronik ve süreğen niteliği olduğu vurgulanmaktadır. Epidemiyolojik veriler, SDB’nin küresel ölçekte yılda yaklaşık 105 milyon kişiyi etkilediğini ve görülme sıklığının kadınlarda (%1,8) erkeklere (%1,3) oranla daha yüksek olduğunu ortaya koymaktadır. Multifaktöriyel bir yapıya sahip olan etiyoloji; genetik yatkınlık, monoaminerjik iletim sistemlerindeki düzensizlikler, erken dönem travmatik yaşantılar, kronik stres etkenleri ve işlevsel olmayan bilişsel şemalar arasındaki karmaşık etkileşimlere işaret etmektedir. Komorbidite açısından bakıldığında, hastaların en az %75’inde ek bir psikiyatrik bozukluğun eşlik ettiği; bunlar arasında majör depresif bozukluk, anksiyete bozuklukları ve kişilik bozukluklarının en sık görülen eş tanılar olduğu bildirilmektedir. Tedavi yaklaşımları hem farmakoterapiyi hem de psikoterapiyi içermekte; birinci basamak farmakolojik seçenek olarak seçici serotonin geri alım inhibitörleri (SSRI’lar) tercih edilirken Bilişsel Davranışçı Terapi ve Bilişsel Davranışçı Sistem Psikoterapisi etkili psikoterapötik müdahaleler olarak öne çıkmaktadır. Mevcut bulgular, farmakoterapi ile psikoterapinin birlikte kullanılmasının (%75 yanıt oranı) tek başına uygulanan tedavilere (%48) oranla daha yüksek etkililik sağladığını göstermekte; bu doğrultuda erken tanı, kapsamlı klinik değerlendirme, kombine tedavi yaklaşımları ve uzun süreli izlem, SDB’nin etkili ve sürdürülebilir yönetimi için kritik bileşenler olarak değerlendirilmektedir.
